  1. Basics & How to Apply: Includes a list of all forms & “why” we go

  2. Team Application: This is a general application & includes questions for medical & ministry purposes

  3. Team Covenant: Incudes agreement to standards for serving people well

  4. Medical Release: In case of an emergency, this medical release is recommended for all members.

  5. Liability Waiver: This form requires a notary for liability.

  6. Background Check: This gives permission for a background check to be completed upon the applicant

  7. Pastor Reference Form: Each member going on mission should have a pastor or local church leader reference
